2004 TEAM NJ SUMMER CLASSIC - JUNE 24-27TH !!!

Please Note: This is an invitational Tournament Only. The forms are for the teams that have already been accepted into the tournament.  Please contact the tournament director if you have any questions.

2004 Summer Classic Entry Form - All teams entering the tournament must complete the entry form.  The fee for entering the tournament is $495.  (Reminder: This is an invitational Tournament - Only teams that have been accepted into the tournament must complete the forms).
