2010 Fall Tournament Entry Form -
All teams entering the tournament must complete the entry form. The fee
for entering the tournament is $650.00. (Reminder: This is an invitational
Tournament - Only teams that have been accepted into the tournament must
complete the forms - do not send a form and check if you
have not been accepted into the showcase). Please complete the form and

a Microsoft Excel file. Please save the form to your computer, complete,
and send as an attachment via email to:
