All teams entering the tournament must complete the entry form. The fee for entering the
tournament is $750.00 for a 5 game guarantee and $950.00 for a 7 game guarantee.
(Reminder: This is an invitational Tournament - Only teams that have been accepted into
the tournament must complete the forms). Please complete the form and remit payment

All teams entering must complete the player profile form. The form is in a Microsoft Excel
file. Please save the form to your computer, complete, and send as an attachment via
email to: teamnjsoftball@yahoo.com. Type over the fields where the data belongs - for
example, where it says "Name" type in the players name.
